Telehealth Consent

This Telehealth Consent (“Consent”) explains how telehealth services are provided through Luvo Health (“Luvo,” “we,” “us,” or “our”) and obtains your informed consent to receive healthcare services remotely from licensed healthcare providers and professional medical entities (the “Professional Entities”) using secure technology platforms.

Purpose of This Consent

Telehealth allows licensed healthcare providers to evaluate, diagnose, and treat patients remotely using electronic communications and information technology. This Consent is intended to help you understand the benefits, risks, and limitations of telehealth and to obtain your consent to receive care in this manner.

What Is Telehealth

Telehealth services may include the electronic transmission of health information, completion of online intake forms, secure messaging, audio or video visits, and other remote communications between you and your healthcare provider.

Potential Benefits of Telehealth

Telehealth may provide convenient access to care without requiring an in-person visit and may allow you to engage with a provider at times and locations that are more convenient.

Alternatives to Telehealth

Alternatives to telehealth, including in-person medical care, may be available. You are not required to receive care through telehealth and may seek care from other healthcare providers at any time.

Limitations and Risks of Telehealth

Telehealth is not a substitute for ongoing, in-person care from a local healthcare provider. Because telehealth encounters do not involve a physical examination and may rely on information you provide, there are inherent limitations and risks, including delayed diagnosis, incomplete assessment, or adverse outcomes.

Technology failures, interruptions, or connectivity issues may delay or disrupt care.

Telehealth is not appropriate for all medical conditions.

Emergencies

Telehealth services are not intended for emergency situations. If you believe you are experiencing a medical emergency or life-threatening condition, call 911 or seek immediate emergency care. Providers may not be able to respond immediately to communications submitted through telehealth platforms.

Provider Discretion and Treatment Decisions

Your healthcare provider will determine, in their professional judgment, whether telehealth is appropriate for your condition. Participation in telehealth does not guarantee that a diagnosis, prescription, or treatment will be provided.

Your provider may recommend in-person care, additional evaluation, or alternative treatment options.

Healthcare services may be provided by nurse practitioners or physician assistants, rather than physicians, as permitted by applicable law, and you consent to receive care from such providers.

Your Responsibilities

You agree to provide complete, accurate, and truthful medical information, including your medical history, medications, supplements, allergies, and emergency contact information. Failure to provide accurate or complete information may negatively impact your care.

You agree to participate in telehealth visits from a private location to help protect the confidentiality of your health information.

Electronic Communication and Privacy

Telehealth services involve the electronic communication of personal and health information. Reasonable administrative, technical, and physical safeguards are used to protect your information, but no system can guarantee absolute security. There is a risk of unauthorized access despite these safeguards.

Your health information will be handled in accordance with applicable privacy laws and privacy policies.

Financial Responsibility and Relationships

You understand that you are responsible for all charges associated with telehealth services, including provider services, prescriptions, and any related costs, except as otherwise required by law.

Luvo may have contractual or financial relationships with the professional entities, pharmacies, or service providers involved in delivering or supporting your care. You are free to obtain medical services or prescriptions from providers or pharmacies not affiliated with Luvo.

Medical Provider Consents and Policies

Medical care is provided by third-party licensed healthcare providers and professional medical entities, not by Luvo. Prior to receiving care, you will be required to review and accept the applicable telehealth consent, notice of privacy practices, and other required disclosures presented by the medical provider or platform delivering your care, including OpenLoop Healthcare Partners, PC or its affiliated professional entities. Those consents and disclosures govern the provision of medical services and are incorporated by reference into this Consent.

Withdrawal of Consent

You may withdraw your consent to telehealth at any time by discontinuing use of the services or by providing notice to your healthcare provider. Withdrawal of consent will not affect care already provided or actions taken prior to withdrawal.

Acknowledgment and Consent

By using Luvo’s services, you acknowledge that you have read and understand this Telehealth Consent, that you have had the opportunity to ask questions, and that you voluntarily consent to receive healthcare services via telehealth under the terms described above.
